Program Membership Agreement
Mobile IV Medics makes available to individuals who register as Members of the Program (“Users”) certain products and services offered by Mobile IV Medics or by third-party medical providers, pharmacies, or other vendors via the Program.
Your membership (“Membership”) in the Program, including your ability to access the Membership Services and your obligation to pay Membership Fees (each as defined below) will commence upon payment of Membership Fees.
Your Program Membership includes use of the Mobile IV Medics website; customer support; pharmacy and clinical care coordination; health information management; and facilitation to access the nurse hotline if you have questions or concerns regarding your prescription(s) or other clinical issues.
If prescribed, medication fulfillment is offered through compounded pharmacies (“Pharmacies”) licensed appropriately to dispense medications to the state jurisdictions which are served by Mobile IV Medics.
You agree and understand that your prescription(s) may be filled by and transferred between any of the Pharmacies, and you agree that Mobile IV Medics and/or it’s affiliates may take these actions on your behalf. Mobile IV Medics does not control or interfere with any professional service provided by the Pharmacies, and each is solely responsible for their provision of professional services rendered via the Program.
By accepting this Agreement, you acknowledge and agree that any services you receive from the Pharmacies, Medical Groups or Providers through the Sites are also subject to this Agreement, and that the Labs, Pharmacies, Medical Groups and Providers are third-party beneficiaries of this Agreement.
Membership Term
All memberships have a required initial Membership Term of three (3) months unless otherwise specified. Following the initial Membership Term, your Membership with converted to month to month. Cancellations requested within the initial Membership term will be required to fulfill all payments (as defined below) required for the initial Membership Term.
Payment of Membership Fee
You will have an opportunity to review and agree to the cost of your Program Membership (the “Membership Fee”) during the checkout process. You may also view the costs associated with Membership on the Program webpage located here. Once your Membership begins, your Membership Fee will be automatically charged to your payment card on file on a recurring, monthly basis (approximately every 28-30 days) until your Membership is canceled. The initial payment must be made the day your Membership commences. Once paid, your Membership Fee is non-refundable. Canceling your Membership
You may cancel your Membership at any time by contacting us here – https://mobileivmedics.com/weight-loss-program-contact-us/. Any Membership Fees paid prior to the date of cancellation are non-refundable.
