MIVM Media Release 

I hereby grant Mobile IV Medics, and any and all employees and/or agents of Mobile IV Medics, the right and permission to use and/or publish photographs and/or videos of me for art and promotional purposes including but not limited to, advertising, publicity, commercial or display of use. Also authorize my photos to be posted on social media, such as Facebook, Twitter, and the office’s website page. 

I hereby release and discharge Mobile IV Medics and all persons functioning under his/her permissions or authority from any legal or equitable claims including but not limited to the following: blurring of the image(s), alteration, distortion or use in composite form, libel, invasion of privacy or any claims based on the production or in the process of recording or publishing the materials. 

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image. 

I have read this release before acknowledging, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to acknowledging, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
